A: Clients are referred to Sunflower Wellness only through participating physicians. Because we are not a healthcare organization, we don't maintain or evaluate patient medical records, and are therefore unable to assess the specific needs or overall health of our clients. Each client's physician is responsible for determining the eligibility of the client to receive services from us.
Please speak with your physician further. At this time, please note, however, that our program is small and growing. We only operate in San Francisco and Marin Counties in California with a limited number of qualified physicians. We are working to expand our operations as of late 2009 and beyond, so please check back on our website for news and updates.
A: We currently restrict the number of physicians who participate to a small number in our pilot program in San Francisco. If you would like to sign up on the website, we would be glad to contact you when we expand our program. Physicians who practice in areas compatible with our mission and services (such as oncologists) may sign up here.
A: At the time we send you coupon books we will try to provide you with an updated list of registered service providers. You can also look on our website here.
A: At this time (second half of 2009) we are working with a limited number of Bay Area service providers, including personal trainers, yoga and movement instructors. To sign up to work with us as we expand our programs, please sign up here.
A: The place for providers to sign up is here. Please note that we currently only work with providers in San Francisco and Marin Counties in California.
A: Reimbursement procedures have been defined differently for our separate programs. For further information, please contact us through the link at the top of this page.
A: No, we do not withhold income taxes from payments to our providers. Our providers are independent contractors, for whom we furnish an IRS Form 1099 at the end of each tax year. It is the responsibility of each service provider to file their income taxes, including income shown on a Sunflower Wellness Form 1099.
A: We send checks out at the end of every month. We are working to improve our cycle time by adding an additional mid-month payment cycle as soon as our resources permit. Our goal is to send checks out twice monthly.
A: We are very interested in hearing from people who want to help. If you're a service provider, you can check out information for service providers here. We are especially looking for people interested in keeping the Exercise Training manual up to date. You can find the book here. If you're interested in helping us expand the Sunflower Wellness program or in volunteering in any other way, please tell us by using our Volunteer page, which is here. Please understand that we are an all-volunteer organization (to date), so we ask for your patience while waiting for a reply.
In any case, we are extremely grateful for any interest that people show in helping us. Thank you!
