We maintain the following in conducting our work at Sunflower Wellness:
1. Sunflower Wellness is a 501(c)(3) nonprofit charitable organization, and is not a healthcare provider, a healthcare insurance company, or a medical provider. We maintain no expertise of any kind for examining or treating Patients.
2. Sunflower Wellness relies solely upon the professional judgment of participating Physicians to determine whether any referred Patient of that Physician should engage in the activities provided by Sunflower and its contracted providers. All services are provided at the Patient's risk. We expect the Physician to inform the Patient as to any and all limitations for the Patient's participation with any of Sunflower Wellness's programs.
3. Sunflower Wellness, including its officers, directors, volunteers, employees and contractors, is not responsible for any injury or harm whatsoever arising from exercise recommendations, coaching or guidance provided by service providers. Although we make every reasonable effort to protect patients at all times, patients engage in our services at their own risk.
4. Sunflower Wellness waives any and all liability for the safety, well-being and health of the Patients who participate in its programs.
5. All services provided by service providers are on a contractual basis only.
6. We require that Physicians provide their patients with a Medical Clearance form to exercise. Once you register with us, you can request medical clearance forms here.
7. Sunflower Wellness does not maintain patient records of any kind, and will not retain copies of its Exercise Recommendations.
8. Sunflower Wellness provides no reimbursement to Physicians for any reason whatsoever. We are prohibited from doing so in order to
maintain our nonprofit status. We are solely in business to provide access for Patients to non-medical services.
9. Finally, despite the need for our specifying these terms and conditions, Sunflower Wellness would like to express sincere gratitude to all Physicians who work with us to help improve the lives of people who really need support and assistance. Thank you.
