We thank you for your interest in working with Sunflower Wellness!
1. At this time, Sunflower Wellness has kicked off its efforts in San
Francisco and Marin Counties in California only, with limited expansion
in the Bay Area. We plan to expand further into California and nationwide
as resources, interest and participants become further available.
2. If you practice medicine in the Bay Area in a practice area compatible
with our mission and the services we provide, please sign up with us here.
3. We will only provide services through the offices of participating Physicians.
4. To refer a patient to Sunflower Wellness, we ask that you do so online
here. Upon receipt of the referral
that includes the patient's contact information, we will contact the patient
to set up an Exercise Assessment Consultation session.
5. We can also supply participating physicians with paper Medical Clearance
forms for exercise. Physicians can request Medical Clearance forms
here.
It is important to give a copy of the form to the patient so they can contact us to make an appointment
for an Exercise Assessment Consultation session. The patient must bring that form to the session, because
we will have no other way to receive your paper-based referral.
6. We have designed the Exercise Assessment Consultation session to determine what movement and exercise
regimens to recommend that will most benefit the Patient.
7. At the end of the Exercise Assessment Consultation session, we provide the Patient
with an Exercise Recommendation. PLEASE INCLUDE YOUR EMAIL ADDRESS ON
THE MEDICAL CLEARANCE FORM, so that we can email you a copy of your Patient's
Exercise Recommendation.
8. The Patient is also directed toward free, low-cost, and at-cost service
providers and exercise classes that will fit their needs, and increasingly,
their geographic location requirements.
