Sunflower Wellness provides exercise and movement to people
living with cancer. The process begins with a physican referral (our
Medical Clearance form), which we ask that you fill out. You can
request a supply of forms using the link below.
We will provide your patients with an Exercise
Assessment Consultation session (about one hour), give a copy of our
Exercise Recommendation to each patient, and return a copy to you
(email preferred!) for your records.

you have not already done so.
